GoHighLevel Appointment Reminder: How It Actually Works

A GoHighLevel appointment reminder that fires once, by email, leaves a lot of no-shows on the table.

The platform's calendar system runs on four channels, email, SMS, WhatsApp and in-app, with six separate notification types covering everything from booking to follow-up. A reminder is just one of those six, and it's the one most practices under-use.

Where a GoHighLevel appointment reminder actually lives

Every calendar has its own Notifications tab, under Calendar Settings. Six message types live there:

  • Appointment booked, while it's still unconfirmed
  • Appointment booked, once it's confirmed
  • Cancellation, including no-shows
  • Reschedule
  • Reminder, sent a set amount of time before the appointment
  • Follow-up, sent once it's over

Each one can go to the contact, a guest, the assigned staff member, or an extra address you add yourself. Timing is set per notification.

The reminder lets you choose how far in advance it fires, so a practice can layer a day-before email with a same-day text without the two colliding. Spacing them out that way is close to what wait steps do inside a workflow: nothing fires until its turn.

SMS needs a valid country code on the contact's number before anything sends. WhatsApp needs an approved template and a verified business account, so it isn't a channel most accounts can switch on for today's appointments.

Where a contact has no verified WhatsApp number, branching with if/else conditions down to email keeps that reminder from failing silently instead of just not sending.

What the wording in the message actually changes

Whether a reminder fires isn't the only variable that moves attendance. A pair of randomised controlled trials run across two periods in the English NHS tested different reminder wording on more than 19,000 hospital patients combined. Changing what the message said, not whether one was sent, changed who showed up.

Message wording testedDid-not-attend rateWhat it told the patient
Existing message (control)11.1%Standard reminder, no added detail
Stated appointment cost8.2%-8.4%Named what the missed slot cost the health system
General cost framing9.9%Raised cost without a figure
Empathy framing10.7%Reminded patients of the impact on others

The trust running these trials estimated that switching from its old message to the cost-specific version would produce about 5,800 fewer missed appointments a year, at no extra cost.

That's a UK hospital system, not a GoHighLevel client, but it's solving the same problem: a reminder that states something concrete outperforms one that just says it'll see you Tuesday.

It's the same problem behind a dental practice's recall and no-show automation we built on GoHighLevel: the sequence and the specifics mattered more than any single channel.

The registration step that has to happen before any of this works

None of these reminders send as SMS without A2P 10DLC brand and campaign registration in place first. The framework verifies the business sending the message, the reason it's sending it, and that the recipient consented, and carriers use that record to decide which traffic actually gets through.

Appointment reminders sit in the same registered category as customer-care messages and account notifications, which is also what covers missed call text back automations. Skip registration and the reminder workflow still runs on schedule. It just has no approved number to send through.

Setting this up alongside the notification settings, rather than discovering it later, is the difference between a reminder system that works from day one and one that quietly fails for the first week.

It's usually the first thing we wire up when we build out workflow automation for a new GoHighLevel account.
